This enrolment is active from the date filled in until 31 January the following year, at which point your child will need to be re-enrolled. One form must be completed for EACH child in your family who is part of Cornerstone's childrens' programs.

Permissions & Consent

Permission to use photos/videos of children *

Tip: Tip: As part of the enrolment process, we ask for consent to use photos/videos of your child for marketing and communications material. When our communications team wishes to use pictures or footage including children, a link will be sent to all parents at Cornerstone with the draft PDF or a folder of images for digital applications. If you've selected “permission, provided draft is viewed” when enrolling your child, it’s your responsibility to check the draft for images of your child. Please respond within 5 days if you would not like your child’s image used. If you are happy for us to proceed with the image then you don’t need to do anything. Photos and videos of children will not be published where we do not have parental consent (you don't need to check the draft if you've opted out of having your child's image used upon enrollment; we'll make sure they're not in there first)
